Hi! I am Amy Ouellette (Phelps), a Doctor of Physical Therapy and graduate of Chapman University in Orange, California. I attended Undergrad at Villanova University in Pennsylvania where I received my BS with a minor in Psychology. I have been treating children for over 19 years and I am certified by the APTA as a CAP-Pelvic provider. I offer a personal touch to physical therapy in Sherwood, OR. At Foundations Physical Therapy, LLC, I am committed to listening to what challenges you or your child face, considering all factors that may be at play, providing research based interventions and supportive educational content to empower you to make positive change. Our Fees
We understand that healthcare is expensive and make every effort to keep our fees low while optomizing the ease and flexibility of providing exceptional physical therapy intervention. Our sessions are 50 minutes in duration and are billed directly from a card on file. Our fees are as follows...
Initial evaluation.......$180
Follow up sessions.....$120 for infants in the clinic
.....$140 for in home visits
Though we do not bill insurance, Superbills are available upon request should you wish to attempt to receive reimburement from your insurance. This is not a guarantee of payment.
